Enrollment Forms

It's as easy as 1-2-3-4:

1. Fill out the Student-Family Application

2. Click on the Child Information Packet

3. Gather all the documents you need, including the child's date of birth and immunization record. 

4. Mail, email completed documents or schedule an in-person appointment to enroll.
